Easily create care packages, care plans and end of life planning that incorporate digital remote monitoring
The Dignio ecosystem is designed to support local authorities that want to engage with NHS integrated care systems on joined-up care, to reduce unnecessary hospital admissions, and provide affordable care packages that keep people safe in their own homes.
Using a module within the Dignio Prevent dashboard, you can host care packages, care plans and end of life plans built around multi-agency support and the wishes of an individual. Then, you can make sure they are accessible to everybody who needs to interact with them, via MyDignio or Dignio Care.
The tasks completed by different agencies are recorded and auditable, helping to verify that commissioned services have been delivered to a high standard. You can also incorporate remote monitoring using MyDignio, Dignio Care and connected Bluetooth devices to empower care staff, support early intervention initiatives, and reduce or eliminate ‘medication support only’ visits.
Dignio is also flexible and cost effective. We can price on a per patient basis or patient population basis, but we never charge by pathway. If your local authority is a Dignio customer and is using our technology for one pathway, then you can use it to support almost any other pathway.
Avoid deterioration and prevent unnecessary hospital admissions
Local authorities will be looking to avoid admissions to hospital, particularly over the winter, when the NHS traditionally experiences very high levels of demand. Hospital admissions for frail, elderly citizens can lead to expensive delayed discharges, deterioration, and poor outcomes that need to be addressed by new care packages or admission to a care home.
Dignio has a proven track-record in working with local authorities to support individuals while avoiding unnecessary admissions. We have been working in Oslo since 2014 and have seen our system deployed at scale to more than 30,000 patients. An early study confirmed reductions in hospital admissions and emergency department attendances, as well as home visits by nurses. This led to an estimated 42% reduction in average costs per end user per year.
In the UK, we have been working on a project funded by Stockport Metropolitan Borough Council that grew out of a desire to reduce pressure on healthcare services over the winter. The project has enabled out-of-hours care provider Mastercall Healthcare to reduce both hospital admissions and bed days, by picking up early signs of deterioration.
Mastercall calculates the project with 900 patients has contributed to £6 million of savings to the healthcare community. Dignio solutions are well received by staff and citizens. The Mastercall intervention received 100% positive feedback from clinicians, residents and family members.
Cost effective medication compliance
Dignio has developed a medication compliance solution for local authorities that want to enable individuals to live an active everyday life, while reducing or eliminating the need for ‘medication support only’ visits. The solution uses digital dispensers that are pre-loaded with their medication and that send reminders to individuals to take it as required.
Relatives, carers and professionals using the Dignio Prevent dashboard receive alerts if medication is not collected. The solution can be integrated into a comprehensive connected care programme, or independently. In Norway, we are working with 180 municipalities on medication projects run by the Norwegian Directorate of Health.
Analysis has found that these have improved compliance with medication plans, saved time for providers who have been able to replace daily visits with fortnightly visits and avoided costs. The municipality of Oslo has reduced home visits for this patient group by 35% and visit time by 38%, while Bergen has reduced home visits by 38% and visit time by 58% (link to report).
Designed for interoperability, connectivity and ease of use, with security and confidentiality built in
The Dignio ecosystem is cloud-based and built to be interoperable with the IT systems that are commonly used by health and care providers. The MyDignio app and Dignio Care are designed for connectivity with a wide range of Bluetooth devices and for simplicity and ease of use: MyDignio app can be set up to submit data to Dignio Prevent at the touch of a button.
At the same time, we are committed to quality, information security, and meeting the standards that councils, providers and users rightly expect. We are CE marked as a medical device and certified against the ISO 13485 standard that supports medical device manufacturers in designing quality management systems.
We are also compliant with the ISO 27001 standard that helps organisations to set up and continually improve an information security management system. And we have passed the NHS Data Technology Assessment Criteria or DTAC.
